What Is a Community Health Improvement Plan?

A Community Health Improvement Plan, or CHIP, is a three-year plan created with community input to:

  • Identify key health priorities and strategies
  • Build on community strengths
  • Coordinate efforts across organizations
  • Improve health and quality of life for all residents

The plan is shaped by local data and, most importantly, by the voices of community members. This information is compiled into a document called a Community Health Assessment. For more information, visit the Community Health Assessment page.


2026-2028 CHIP Priorities

Based on community input and data, the plan focuses on five priority areas:

  • Access to Healthcare
  • Safe Drinking Water
  • Food Insecurity
  • Housing
  • Mental Health

Each priority includes goals and objectives that partners will work on together.


How You Can Get Involved

Improving community health takes all of us. You can get involved by:

  • Attending events and coalition meetings
  • Partnering on actions that support the plan
  • Sharing your ideas
  • Helping spread the word about local resources
