Preparing for Your Visit
Step 1: Before calling to schedule your appointment, have the following information ready:
- Name
- Date of Birth
- Address
- Phone Number
- Name of Insurance Carrier
- Type of Immunizations Needed (if known)

Step 3: Before arriving for your appointment
To expedite the registration process, you may bring the following paperwork to your appointment.
- Print and complete your Registration Form (English)
- Print and complete appropriate Screening Form
If a legal guardian is not accompanying a minor child, this completed form must be presented at the time of the appointment.
- Minor Child Letter of Authorization* (if applicable) forms for each patient (English)
Step 4: Arrive five minutes prior to your appointment to complete the registration process
Bring the following items with you:
- Completed Registration, Screening, and Letter of Authorization* forms for each Family Member
- Immunization Records
- Insurance Card
*Any minor (0-17yrs) being accompanied by a family relative or friend will require a letter of authorization from parent or legal guardian (See Letter of Authorization Form).
No child will be denied childhood immunizations because of an inability to pay.
